New Patients

Health Intervention Services is a non profit health clinic serving the uninsured in Grand Rapids.

Qualifications for new patients:

	No Health Insurance, Medicare or Medicaid
	Proof of income for everyone in the household (such as a paycheck stub).
	Minimum payment of $15 per office visit plus additional fees for lab work and some procedures.
	It may take up to 6 weeks for a new patient appointment.

Hours: Mon. 8:30am-5 pm and 6-8pm, Tues. thru Thurs. 8:30am-5pm, Fri. 8:30am-1 pm. Closed daily Noon to 1 pm.
